Trainer Card Help

I am frequently seeing the dope custom trainer cards with the trainer's name, FC, team, and a set of badges on it and I really want one... If anybody knows a good site to make em or can make one themselves plz help me!
Oh and the site/ person has to be able to show genders and shiny forms!
